Ohana Mental Health provides services to individuals physically located in Minnesota at the time of their appointment, by providers licensed in Minnesota. Telehealth availability outside Minnesota, where applicable, will be stated separately and is limited to states where our providers are licensed.
This website and its forms are intended for use by individuals 18 and older. If you are under 18, a parent or legal guardian should submit forms and appointment requests on your behalf.
Appointments requested through this website are subject to confirmation. Telehealth visits require a stable internet connection and a private location, and are subject to separate telehealth consent provided at the time of your first visit. Cancellation and no-show policies are provided separately at the time of scheduling.
Contact forms and email are not secure or encrypted methods of communication and should not be used to send clinical information, urgent requests, or anything you consider sensitive. These channels are not monitored continuously. For urgent matters, call our office directly; for emergencies, see Section 3 above.
If you opt in to receive appointment reminders or other communications by text message, message and data rates may apply, and message frequency varies. Reply STOP to opt out at any time, or HELP for assistance. Mobile opt-in information and consent are not shared or sold to third parties for marketing purposes.
If you are given access to a client portal, you are responsible for maintaining the confidentiality of your login credentials and for all activity under your account. Notify us immediately if you suspect unauthorized access.
