1. Acceptance of Terms

By accessing or using the Ohana Mental Health website (suprtapp.com), you agree to these Terms of Service. If you do not agree, please do not use this website.

2. About This Website

This website provides general information about Ohana Mental Health’s services, providers, and how to schedule an appointment. It is not a substitute for professional medical advice, diagnosis, or treatment, and using this website does not create a provider-patient relationship. Our HIPAA Notice of Privacy Practices and Accessibility Statement are incorporated by reference and available on this website.

3. Not for Emergencies

This website is not monitored for emergencies. If you are experiencing a medical emergency, call 911. If you are experiencing a mental health crisis, help is available:

4. Eligibility and Geographic Availability

Ohana Mental Health provides services to individuals physically located in Minnesota at the time of their appointment, by providers licensed in Minnesota. Telehealth availability outside Minnesota, where applicable, will be stated separately and is limited to states where our providers are licensed.

This website and its forms are intended for use by individuals 18 and older. If you are under 18, a parent or legal guardian should submit forms and appointment requests on your behalf.

5. Scheduling and Telehealth

Appointments requested through this website are subject to confirmation. Telehealth visits require a stable internet connection and a private location, and are subject to separate telehealth consent provided at the time of your first visit. Cancellation and no-show policies are provided separately at the time of scheduling.

6. Website and Email Communications Are Not Secure

Contact forms and email are not secure or encrypted methods of communication and should not be used to send clinical information, urgent requests, or anything you consider sensitive. These channels are not monitored continuously. For urgent matters, call our office directly; for emergencies, see Section 3 above.

7. Text Message (SMS) Communications

If you opt in to receive appointment reminders or other communications by text message, message and data rates may apply, and message frequency varies. Reply STOP to opt out at any time, or HELP for assistance. Mobile opt-in information and consent are not shared or sold to third parties for marketing purposes.

8. Client Portal

If you are given access to a client portal, you are responsible for maintaining the confidentiality of your login credentials and for all activity under your account. Notify us immediately if you suspect unauthorized access.

9. Acceptable Use

You agree not to use this website to:

  • Violate any applicable law or regulation
  • Impersonate another person or misrepresent your identity
  • Attempt to gain unauthorized access to any part of the website or its systems
  • Interfere with the website’s normal operation

Nothing in these Terms limits any right or remedy you may have relating to the professional health care services you receive from Ohana Mental Health or its providers.

14. Governing Law and Venue

These Terms are governed by the laws of the State of Minnesota, without regard to conflict of law principles. The state and federal courts located in Hennepin County, Minnesota have exclusive jurisdiction and venue over any dispute arising from these Terms or your use of this website.

15. General Provisions

If any provision of these Terms is found unenforceable, the remaining provisions remain in full effect. These Terms constitute the entire agreement between you and Ohana Mental Health regarding use of this website. Our failure to enforce any provision is not a waiver of that provision. You may not assign these Terms; we may assign them in connection with a merger, acquisition, or sale of assets. These Terms do not create any third-party beneficiary rights. We reserve the right to suspend or terminate your access to this website at any time, for any reason.

16. Changes to These Terms

We may update these Terms from time to time. Continued use of the website after changes are posted constitutes acceptance of the updated Terms.
